Frank Polochak

POLOCHAK Frank Polochak Jr., 66, passed away on April 14, 2013. Born in Queens, NY on June 2, 1946. He proudly served his country in the U.S Navy, and later was employed by Shands Jacksonville Hospital. Frank was a member of the American Legion, Chapter 129. He loved fishing, the outdoors, landscaping and gardening. Survivors include his mother, children and many grandchildren that loved him dearly. Visitation will be from 6:00 to 8:00 p.m., Thursday, April 18th in Quinn-Shalz Funeral Home of Jacksonville Beach.
